How It Works

  • The patient’s data is securely transmitted to Omedix servers.
  • Your practice is notified.
  • You are then free to login to view the form request.

All requests are HIPAA-compliant.1 Omedix SecureForms can be appointment requests, prescription renewals, bill payment, or virtually any other form you’d like to create!

1: Naturally, there are some things you have to do on your end to maintain HIPAA-compliance. This is explained in more detail in our service contract.

Omedix SecureForms is a PortalPlus™ Module!

Omedix SecureForms isn’t just a standalone service; it’s actually a module of our PortalPlus™ platform. That means that when you add services in the future like eNewsletters, patient surveys, a practice blog, secure messaging, automated appointment preparation, or any other number of modules, it’s integrated with your SecureForms.

Example: After requesting an appointment, your patient can login to your website and see a history of all appointments he requested, and chose to sign up for a special newsletter when requesting an appointment.

What's Included?

  • Customization
    We will customize your online forms, whether an appointment request form, a prescription renewal form, a registration form, a medical history form, or any other. The end user will be able to fill out the form and submit it over an encrypted connection.
  • Email Notification of New Form Submissions
    At your request, any member of your staff can receive an e-mail notification when a particular form is submitted.
  • Secure and Personalized Login for Each Member of Your Staff
    Each member of your staff will have a secure and personalized login to process any form submissions.
  • Archive of All Forms Submitted
    It is impossible to delete a form submission from the database. When processed, forms are “archived” for future searching if necessary.
  • Daily Backup of All Data
    All database files are backed up to an offsite location on a nightly basis.
